Comprehensive Health Assessment Plan

$275.00/year +HST

Children must have a minimum of 1 adult with a membership plan

What the CHAP Membership Includes:

  •  1-hour initial clinical appointment and annual physical assessment

​

  •  Review of all healthcare needs

​

  •  Preventative screening

​

  •  Diagnostic tests

​

  •  Referrals

​

  •  Forms

​

  • Unlimited follow up appointments

​

  • Access to urgent care services

Is the CHAP Membership right for you?

  • Membership is ideal for individuals with ongoing health issues

    • ie: Hypertension, heart failure, COPD, Diabetes, mental health

​

  • We recommend membership for individuals without a primary care provider

​

  • Anyone seeking ongoing prescription renewal must be a member

​

  • GreenNPC encourages anyone seeking reliable, regular access to healthcare services to register for the CHAP membership

Maintaining your Membership

  • All clients are independently responsible for maintaining their membership

 

  • Membership renewals are available 3 months prior to end of term

​​

  • Members will be reminded to renew their membership annually. Failure to renew a membership will result in termination of care

​

  • We encourage all members to fully review the Termination Policy

​

  • Membership is not limited to any one provider. At GreenNPC you are a member to the clinic. Clients may prefer to see the same provider, but will be offered access to another provider in the event their main provider is not available

Caring for you and your family

  • As of Nov 2025 we are excited to announce the start of Phase 2​

​

  • Phase 2 starts to see the introduction of dependent services.

​​

  • Dependents must have 1 guardian/ parent register an adult membership. 

​​

  • The dependent's membership terms are subject to the Membership and Termination Policy of the Adult Membership plan

​​

  • By Phase 3 of operations there will be fully expanded access to services for dependents.

Once booked, you will receive a notification to download the TelusCHR

All future bookings are managed in the CHR free of further charges.

What is a CHR?
A Collaborative Health Record is a formal tool utilized by health care professionals for documenting and recording sensitive health related information.

While there are several of these systems available for health care professionals, we chose the TelusCHR as is aligns with our mission to enhance patient engagement, giving patients an opportunity to communicate directly with their health care team members, see test results, and request appointments; ultimately allowing patients to take a more active role in their healthcare.
