An active OHIP card is recommended, but not required. Clients should be aware, if they do not have an active OHIP card, there are significant limitations or additional costs for available health care services, including diagnostic tests and referrals.
The UCC is for Non-life threatening conditions only. If you think you have a life-threatening condition please go to your nearest ER.

There is currently a wait time of approximately 2 months for Form completion (ie: ODSP, VAC forms, Disability tax credit)​

Anyone seeking disability management must be a member

​Non-urgent Referrals may be denied with the UCC access fee

Referrals require our NPs to remain professionally responsible for managing the context of the interprofessional relationship. In most circumstances, this time and commitment requires a membership

MTO License Reinstatement Requests

Our NPs require any person seeking to have a drivers license reinstated to be a Member for a minumum of 365 days
-
Additional requirements can be expected based on the nature of the MTO Notice

The website scheduling fee applies to both members and non-members. As per our cancellation policy, the $10 booking fee is non-refundable. The full access fee is due after your appointment.
