Membership is ideal for individuals with ongoing health issues

Anyone seeking ongoing prescription renewal must be a member

GreenNPC encourages anyone seeking reliable, regular access to healthcare services to register for the CHAP membership
Maintaining your Membership

All clients are independently responsible for maintaining their membership
-
Membership renewals are available 3 months prior to end of term

Members will be reminded to renew their membership annually. Failure to renew a membership will result in termination of care
​

We encourage all members to fully review the Termination Policy
​

Membership is not limited to any one provider. At GreenNPC you are a member to the clinic. Clients may prefer to see the same provider, but will be offered access to another provider in the event their main provider is not available
Caring for you and your family

GreenNPC plans to offer services to dependents. It is anticipated that in the first year of operations, there will be very limited access to care for dependents.

During Phase 1, dependents may be encouraged to access services via our walk-in clinic. Drop-in rates will apply.
​

Phase 2 will start to see the introduction of dependent services; by Phase 3 of operations there will be fully expanded access to services for dependents.
